M.M. London Gazette 14 December 1916, at which point his number was listed as '550'. This error was corrected in the London Gazette of 29 July 1921.

Sidney Humphries/Humphreys served with the 1st, 2nd and 17th Battalion, Royal Welch Fusiliers and was noted as an Acting Sergeant.
